Village Residents wishing to file a complaint regarding a violation of the Village Code or NYS Building Code may download the form below and mail it or email it to Village Officials.  Complaint forms can also be mailed to residents upon request, request a form to be mailed by calling Inspector Ken Pretto at 516-790-5713. 

Residents who wish to obtain status of complaints must provide their telephone number or mailing address on the complaint form so that we can contact you after a determination has been made.
